How to Apply for Access-a-Ride

There are 4 components to the Access-a-Ride application:

  1. A written application
  2. A medical verification form to be completed by the licensed professional most knowledgeable about your condition
  3. An In-person Interview
  4. A Transportation Assessment*

*RTD can provide transportation to this appointment.

The completed application may be faxed to 303-299-2169 OR mailed to (please make sure that pages printed double sided are faxed double sided):

Once RTD has reviewed the completed documents, they will be forwarded to our assessment center, and you will be contacted for scheduling via an automated call system. If it has been two weeks since the application was submitted and you have not heard from us, please feel free to contact us at 303-299-2960.

Applications not completed within 90 days of submission will be considered withdrawn.

Important note: All determinations are made based on the information gathered from your application, medical verification form, and your assessment.

Applicants who wish to appeal their eligibility determination will be provided with information on how to make an appeal when their assessment results are mailed.
